Note: Many spaces in the Harry W. Bass Jr. School of Arts, Humanities, and Technology have special equipment and security measures. For example, Jonsson Performance Hall (JO 2.604) contains a $50,000 piano. By submitting this form you agree to take full responsibility for the use and security of the space and equipment during the time requested. (You may be asked to attend a training session before you are granted access to equipment.)

Note 2: If you are requesting space in Jonsson Performance Hall and require podium access and equipment (such as a DVD player, microphone, projector, PowerPoint, etc.), you must contact media services at media@utdallas.edu or call 972-883-4900 in addition to filling out this form.
